Vietnam Visa On Arrival Form

The Vietnam Entry and Exit Form (NA1), also known as the Vietnamese visa application form, is a crucial document that all passengers must submit to Immigration Officers upon entering Vietnam.

Here’s how it works:

  • Form Distribution: Typically, this form is handed out on the airplane or upon landing at a Vietnam airport.
  • Completion: Fill out the required information on the form and return it to the Vietnam Immigration Officer at the airport or border gate.
  • Visa Stamping: If you have a Vietnam Visa on Arrival, the officer will stamp the visa into your passport using your visa approval letter. If you have a visa from the Vietnam Embassy (pre-arranged visa), the officer will give you an entry stamp.

To save time and avoid any issues at the airport (e.g., no pen or glue to attach a photo), it’s advisable to download the form, complete it in advance, and attach a passport-sized photo before your trip.

Instructions for Filling Out the Vietnam Entry and Exit Form (NA1):

  1. Full Name: Write in capital letters, exactly as it appears in your passport. Example: ANDREW RICHARD CROSTIE.
  2. Date of Birth: Use the format dd/mm/yyyy.
  3. Sex: Indicate your gender (male or female).
  4. Place of Birth: Write both the city and country of birth.

5 & 6. Nationality at Birth/Present Nationality: List both if applicable. Example: United States / British (not American or British).

  1. Religion: Write the name of your religion if applicable.
  2. Current Occupation/Profession: Match this with what you provided to the Vietnamese embassy or simply state your current occupation.
  3. Employer and Employer’s Address: Provide the name and address of your employer.
  4. Home Address (Permanent Address): Your home address in your country of origin. Include your phone number and email address.
  5. Family Members: Provide details for each family member, including Full Name, Date of Birth, Nationality, and Permanent Address.
  6. Passport Details: Include your passport number, type (standard/diplomatic/official), date of issue, and expiry date. For “Issuing Authority,” list the place where your passport was issued; check your passport for details.
  7. Date of Latest Entry into Vietnam: Enter the nearest date of your last exit from Vietnam.
  8. Proposed Date of Entry: Your estimated entry date into Vietnam. Also, indicate the proposed length of stay (e.g., 20 days or 2 months).
  9. Purpose of Entry: Select the purpose (tourism/business/study/work/other). Provide the name and address of your contact in Vietnam, such as a hotel for tourists or a company for business purposes.
  10. Proposed Temporary Address in Vietnam: Write your intended address in Vietnam (e.g., hotel address, organization address).
  11. Hosting Organization or Individual in Vietnam: Provide the name, address, and relationship of the host organization or individual.
  12. Accompanying Children Under 14 Years Old: List any accompanying children.
  13. Entries Requested: Specify the type of visa you are applying for (Single entry/Multiple entries).
  14. Other Requests: Note any special requests if applicable.
  15. Done at: Indicate the name of the airport or border gate through which you are entering Vietnam and sign the form.
